Real time alert

Our Complex Event Processing (CEP) engine currently reads ADT (Admit Discharge Transfer), Order, SIU (Scheduling Information Unsolicited) and Result feeds from both adult and children’s hospitals. For an alert to trigger a clinical criteria must be met. For example, one of the earliest alerts used was to screen patients for a clinical trial in the use of prophylactic antibiotics in the management of dog bites. Criteria for this alert consisted of patients aged 18 and over visiting the emergency department with animal/dog bite. This alert would be triggered by the engine whenever an ADT message is received with patients whose date of birth in the PID (Patient Identifier) segment  is 18 or more years ago, with Emergency as the department in PV1 (Patient Visit) segment and DG1 (diagnosis) segment containing dog/animal and bite keywords.

During the early days of the pandemic, CEP alert was used for a number of COVID-19 trials. One of note is a ED department led Eli Lilly randomized, placebo-controlled, phase 2 clinical trial to evaluate the efficacy and safety of LY3819253 (Bamlanivimab) and LY3832479 (Etesevimab) in participants with early symptoms of COVID-19. For this study, CEP generated ~11,500 alerts for ~10,000 unique patients in the last 10 months. For comparison, the seasonal influenza flu alert (Fall-Spring) generates approximately 400 alerts every season.


The name Complex Event Processing is derived from the underlying Esper technology, the software back in the days was able to process large volumes of incoming messages or events, regardless of whether incoming messages are historical or real-time in nature. Now there are multiple stream processing options including the original Esper and the original software has continued to deliver. Over time, CEP engine has supported many different types of clinical trial enrollments. The architecture is described in an early publication.
